When applying for a passport for the
first time, you will need:
- A completed passport application form (available at any U.S. Post Office)

- An official birth certificate with a raised seal (if you do not have
an official birth certificate contact your state government
to learn how to order one.)
- A photo ID (driver's license would do)
- Two official passport photos (these photos must meet federal specification
and can be taken at the Post Office)
- Instructions for submitting the application are on the form
